About this resource

This template captures what the next coordinator needs to know: the participant's preferences and communication, goals and plan dates, current supports, open actions, risks and safeguards, key contacts and how the participant would like to be introduced.

Purpose

To ensure continuity for participants when support coordinators change.

Who it is for

Primary profession: NDIS providers and support coordinators. Also relevant for social workers.

Client or participant group: Support coordinators and provider managers.

Practice setting: Staff changes, leave cover and transfers between support coordination providers.
