About this resource

When life changes, supports often need to change too. This checklist guides support coordinators through common changes, such as moving, health changes, changes in informal support or a crisis, and covers who needs to know, the impact on the plan, service agreements and next steps.

Purpose

To respond promptly and completely when circumstances change.

Who it is for

Primary profession: NDIS providers and support coordinators. Also relevant for social workers.

Client or participant group: Support coordinators working with participants whose circumstances have changed.

Practice setting: Support coordination.

Suggested uses

  • Responding to changes
  • Crisis support
  • Supervision and file review
  • Training new support coordinators

Instructions for practitioners

  1. Talk with the participant about what has changed and what they want.
  2. Check whether a plan change or reassessment may be needed under current NDIS processes.
  3. Update providers with consent.
  4. Record actions and follow up.

Accessibility considerations

Offer meetings in the participant's preferred format.

Scope and limitations

A practice checklist. It is not an official NDIS document.

Research basis

Case management practice standards describe assessment, planning, coordination, review and planned exits with the person at the centre. Person-centred planning shows positive but limited evidence of improving community participation and everyday choice for people with intellectual disability. This resource is designed to align with the NDIS Practice Standards and Code of Conduct; using it does not by itself demonstrate compliance.

What is included

A 2-page printable A4 PDF, including a practitioner notes page with the research basis (version 1.0).
