About this resource
Team meetings can drift into a series of separate updates. This template structures preparation and notes: how the person's views were gathered, updates linked to shared goals, decisions, actions with owners and an accessible summary for the person.
Purpose
To make team meetings focused, person-led and actionable, with updates linked to shared goals rather than discipline lists.
Who it is for
Primary profession: Allied health and multidisciplinary teams. Also relevant for social workers, NDIS providers and support coordinators.
Client or participant group: Multidisciplinary teams, the person they support and the person's chosen supporters.
Practice setting: Case conferences, team reviews and discharge or transition meetings.
Suggested uses
- Regular team reviews
- Case conferences with families
- Discharge and transition planning
- Recording decisions consistently across a team
Instructions for practitioners
- Gather the person's views before the meeting and decide how they will take part.
- Ask each discipline to link updates to a shared goal.
- Record decisions and the reasons for them.
- Assign each action to a person and a date.
- Share an accessible summary with the person, and record only information the team needs.
Accessibility considerations
Plan how the person will take part: in person, by video, through a supporter or by sharing views beforehand. Provide the summary in plain language or Easy Read.
