About this resource

Accessibility considerations
Clear tables with room to write. The Word version can be filled in on screen.
Scope and limitations
An editable template and practical record. It is not legal advice and does not by itself show that a provider meets the NDIS Practice Standards. Check it against the current Practice Standards, NDIS rules, your registration groups and your state or territory's laws, and adapt it to how your service really works.
Research basis
From 1 July 2026, providers of supported independent living must be registered (class of support 0138) and meet the Core Module and the NDIS Commission's SIL Practice Standards: supported decision-making, safeguarding, practice governance, and agreements about tenancy, housing and support. This resource is organised around those standards; using it does not by itself demonstrate compliance. The NDIS worker screening rules require records of risk assessed roles and of each worker's screening check, kept for 7 years, and the Practice Standards require documented induction (including the NDIS worker orientation module), training, infection control training and supervision.
