About this resource

Reviews work best with a clear summary. This template brings together the data period, trends in behaviours of concern and positive behaviours, quality of life indicators, how consistently strategies were used, the person's and family's views and recommendations.

Purpose

To present data clearly so reviews focus on what has changed and why.

Who it is for

Primary profession: Positive behaviour support practitioners. Also relevant for NDIS providers and support coordinators.

Client or participant group: Behaviour support practitioners preparing for reviews.

Practice setting: Behaviour support plan reviews and team meetings.

Suggested uses

  • Plan reviews
  • Team meetings
  • Reports to families and teams
  • Supervision

Instructions for practitioners

  1. Summarise data honestly, including where change is unclear.
  2. Include quality of life, not only behaviour.
  3. Consider implementation before judging strategy effectiveness.
  4. Include the person's and family's views.

Accessibility considerations

Use simple graphs and plain language for the person and family.

Scope and limitations

A summary template. It does not replace required reporting, including any reporting of regulated restrictive practices.

Research basis

Understanding what happens before and after behaviour is the basis of functional assessment, which guides support. Positive behaviour support aims to improve quality of life and reduce behaviours of concern by understanding their function and changing environments and skills.

What is included

A 2-page printable A4 PDF, including a practitioner notes page with the research basis (version 1.0).
